A 46-year-old woman has been recently hospitalised with jaundice and new ascites. She is re-admitted to hospital after 21 days with a creatinine of 1.9 mg/dl (168 µmol/l) and with confusion. A diagnostic ascitic tap is performed and reveals 700 white cells with 60% neutrophilia.
