This ERN RARE-LIVER joint Webinar was held on 17 June 2025 with the PSC and Transition Working Groups.
Transition is a very important topic for HCP especially for HCP dealing with chronic diseases which can have onset in children or young people. The transition is a progress of the young people getting adolescent, therefore something which has to be dealed with from every young people. But in young people this is even more complicate due to the chronic disease, the need for treatment and medical appointments and to “being different”. If the young people are not supported by family and HCP during this transition period and they have a bad transfer (= change of HCP from paediatric to adult services) the risk for non-adherence is very high. ERN Rare Liver are perfect places to discuss about and improve Transition since Adult HCP, paediatric HCP and patients are united in the ERN Rare Liver.
