This ERN RARE-LIVER joint Webinar was held on 17 June 2025 with the PSC and Transition Working Groups.

Transition is a very important topic for HCP especially for HCP dealing with chronic diseases which can have onset in children or young people. The transition is a progress of the young people getting adolescent, therefore something which has to be dealed with from every young people. But in young people this is even more complicate due to the chronic disease, the need for treatment and medical appointments and to “being different”. If the young people are not supported by family and HCP during this transition period and they have a bad transfer (= change of HCP from paediatric to adult services) the risk for non-adherence is very high.  ERN Rare Liver are perfect places to discuss about and improve Transition since Adult HCP, paediatric HCP and patients are united in the ERN Rare Liver.

Marianne Samyn

 

Marianne Samyn is a Paediatric hepatologist working at King’s College hospital and clinical lead for the young adult liver clinic looking after young people aged 16-25 years. Research interest are in longterm outcomes of childhood liver diseases, autoimmune liver disease and adolescent care.
Norman Junge

 

Norman Junge studied human medicine at the Hannover Medical School and began his specialist training at the University Hospital Göttingen. Since 2009, he has been working at the Hannover Medical School in the working group for pediatric gastroenterology, hepatology and liver transplant. Norman Junge’s clinical and scientific focus is on hepatology and liver transplantation, in particular autoimmune hepatitis, gene therapy of congenital liver diseases (in particular Crigler Najjar, member of the CureCn consortium), transition and cardiomyopathy in liver cirrhosis. Since 2020, Norman Junge has been an elected member of ESPGHAN’s Hepatology Committee.

Fabiana Zingone

Fabiana Zingone is an Associate Professor at the Gastroenterology Unit of the Azienda Ospedale–Università di Padova, Italy. Her clinical and research work focuses on immune-mediated gastrointestinal disorders, with particular expertise in celiac disease and inflammatory bowel disease (IBD).  Prof. Zingone is a board member of the Italian Society of Gastroenterology and Digestive Endoscopy (SIGE) and contributed to the recent Italian guidelines for the diagnosis and management of celiac disease. She is also the Treasurer of the International Society for the Study of Celiac Disease (ISSCD).  She works at the Gastroenterology Unit, which hosts a tertiary referral center for inflammatory bowel disease. The center manages the diagnosis and follow-up of over 3,000 patients. Among her clinical responsibilities, she also leads the transition clinic, supporting adolescent patients as they move from pediatric to adult care.
Deepak Joshi

Deepak Joshi is a transplant hepatologist with an interest in autoimmune liver disease. He is the clinical lead for HPB medicine and an advanced biliary endoscopist, specialising in paediatrics. He is a member of the Young Adult Liver service at King’s working in a multi-disciplinary team. He is a Reader in HPB medicine at King’s College London. He is a co-author of the book, Hepatology at a Glance. His current research interests include the management of biliary disease pre- and post – liver transplantation including the role of endoscopy. He is the current secretary of the (BASL) British Association for the Study of Liver.
