A patient with cirrhosis Child-Pugh B is hospitalized for ascitic decompensation. Upon entering the ward, a serum creatinine of 1.4 mg/dL (123 μmol/L) is noted. His last outpatient visit was 7 days earlier. At that time serum creatinine was 0.7 mg/dl (61 μmol /L).

The medical student on the ward asks you whether she may use the term ‘Acute Kidney Injury’ (AKI) in this situation.

A. The diagnosis of AKI cannot be made because serum creatinine values in the previous 3 days are missing

B. The diagnosis of AKI is correct as the serum creatinine of 1.4 mg/dL (123μmol/L) is >50% higher than the value at the last outpatient visit

C. The diagnosis of AKI should not be made because the serum creatinine is below 1.5 mg/dl (132 μmol/L)
